HPLC2019 Kyoto - Information

Those who want to receive the Entry VISA to Japan should ask the Chair, Koji Otsuka,
to make supporting documents after completing their registration w/ payment of the registration fee.

For preparing the supporting documents, following are required.

  1. Full name [clearly showing first & family names]
  2. Gender [female or male]
  3. Date of birth
  4. Nationality
  5. Exact title of job or student status
  6. Full postal address
  7. Dates of entry & departure (to/from Japan)
  8. Address in Japan [hotel name etc.]
  9. Registration number of HPLC2019 Kyoto

10. Proof of air ticket reservation or copy of e-ticket
11. Proof of hotel reservation
